Registration for Fall 2024-2025 is Now Open for all Current Families! 
 

Please print the registration form below and remember to complete each space and sign/date before returning. The annual registration fee is $70 for all programs and is due with the registration form. Forms can be emailed to director@salemchildrenscenter.org when complete.

The health inventory with immunizations form (which must be completed and signed by the child's doctor), the emergency form, the registration form, photo permission form, and a signed copy of "A Parent's Guide to Regulated Child Care" must be on file before your child can enter the program.
